Acute Visits
Acute visit appropriate for primary care to address non-chronic, short-term health issues that can be evaluated and treated same day or within 1 business day such as sore throat, earache, rash, or minor cuts and burn. For medical emergencies call 911 or go or get transportation to the nearest emergency department.
Weight Management
Weight management support: Optimizing lifestyle changes, and when clinically appropriate, we can prescribe weight loss medications. Medication cost and nutrition coach access is discounted but not included in the basic membership only in the Weight Management Complete and first two months Concierge memberships.
Chronic Disease
Prevention Management
Personalized plan to improved general health and prevent diseases like obesity, high blood pressure, abnormal cholesterol, diabetes, and cardiovascular risk reduction. Comprehensive plan based on extensive cardiovascular and diabetes labs results and personalized recommendation available with concierge services.
Virtual Care
Video and phone visit are available when appropriate. After hours visits are available at the discretion of the physician. This does not include medical emergencies. For medical emergencies call 911 or go or get transportation to the nearest emergency department.
Perimenopause and Menopause Management
supporting patients through hormonal shifts, symptom relief, and long-term wellness planning. We center your lived experience—whether you're navigating hot flashes, mood changes, or cycle changes—with affirming, personalized strategies. Labs and treatment are available additional with membership at a discount rate.
Chronic Disease Management
Updated yearly education and tailored plan for obesity, diabetes, high blood pressure, abnormal cholesterol, and mental health. Additional visit might be needed to reach goals. Comprehensive personalized recommendation based on result from extensive cardiovascular and diabetes labs complementary in concierge services.
Hospital Follow Ups
We offer timely post-discharge evaluations to support recovery and reduce readmission risk. These visits include but not limited to medication reconciliation, review of discharge instructions, coordination with specialists, and personalized care planning. This service is available in all plans.
I-693 Exams
Dr. Sarai Ambert-Pompey is a Civil Surgeon able to complete the medical exam for form I-693, review and coordinate necessary labs and immunizations at a discounted price with our community partners. The exam is by appointment only in a private, welcoming environment.

For hospitalizations, emergency care, or catastrophic health events, we strongly recommend maintaining health insurance coverage or pair DPC with cost sharing group. DPC works best as a complement to insurance—not a replacement—offering personalized, accessible care for everyday health needs.
